In consideration of the acceptance of my club fees for the North Shore Distance Running Club, I, the undersigned participant, do hereby for myself, and my heirs, executors, administrators and assigns, forever waive, release and discharge any and all rights, claims, and actions for damages that I or any of us may have, or that may thereafter accrue to us or any of us, against any and all persons, organizations and entities associated with the Club, including but not limited to the North Shore Distance Running Club, any and all sponsors of the Club, the cities and counties in which we participate, individual club members and their respective families, directors, employees, agents, representatives, successors and assigns, arising out of or in connection with my membership. I attest and verify that I am physically fit and sufficiently healthy enough to participate in this organization. I also give my permission for the free use of my name and/or pictures in broadcasts, telecasts, newspapers, newsletters, organization website, etc..
